Checklist

To be prepared for your appointment, make sure:

• Your document is NOT signed. The signing must be witnessed by the notary.
• You have a valid form of identification with you. (See list of valid ID options below)
• You have an accepted form of payment. (Cash, Zelle, Venmo, or Paypal)
